The service you are booking may require you to share sensitive personal information relevant to the treatment (for example, information about a health condition, allergy, or injury), so the provider can deliver the service safely and appropriately.

• You understand any sensitive information you provide will be shared with the specific provider or practitioner fulfilling this booking, for the purpose of delivering that service.
• You consent to Kinjie collecting and sharing this information with that provider for this purpose.
• You understand the provider is responsible for how they handle this information once they receive it, as described in our Privacy Policy and the Data Processing Agreement they accepted to offer this service.

You are not required to share more sensitive information than the provider reasonably needs to deliver the service safely. If you have concerns about a specific question asked of you, contact the provider directly or Kinjie at [email protected].

You can withdraw this consent at any time before the service begins by cancelling the booking. Withdrawing consent does not undo sensitive information you have already shared with the provider, and the provider remains responsible for that information as described above.

Where this consent is made available in more than one language, the English version governs if there is any conflict or inconsistency between versions.
